Name change ceremony

Does anyone have a copy of the offcial name changing ceremnoy that has been discussed here in the past? I cannot locate it and have to prepare for this ritual in the next couple of days.
 
The one thing that I (faintly) remember is that a lot of good champagne is a must...
 
Couple years ago we were at a name change ceremony for a friends boat; went thru the whole ritual including many toasts. The boat; however, was 75 miles away; and I remarked that I thought the boat should also be present. When he went to start his engine that spring; he found it was seized and needed replacing. So maybe it was bad karma, to have the ceremony without the boat? Who knows?
 
Here is what I have done;
Pre-ceremony: Remove existing name / apply new name
Ceremony: kneel, thank God for everything that he has done for me and thank him for allowing me to afford this life style.
Post-ceremony: fire them up and head out into the sunset
